白血病を標的とした新たな免疫細胞療法の開発研究

Research Abstract

自己の免疫細胞を利用する抗腫瘍療法の効果が限定的であるのに対し、同種免疫細胞による抗腫瘍効果は明確に示されてきた。本研究では、同種移植に必要なドナーをHLA不一致血縁者に求め、安全で効果的な治療を行い得るか否かについて検討した。ミニ移植の概念に従った前処置により移植関連毒性を減らし、これとCampath1Hを用いた免疫抑制を統合する方法をとった。Campath1Hは多くのT細胞と一部のB細胞が発言しているCD52に対するモノクローナル抗体であり、ホスト・ドナー双方の免疫を強く抑制するため、HLA不一致ドナーからの移植であっても安定的なドナー骨髄・免疫キメラが期待できる。
これまで白血病4症例に対してHLA不一致移植を行い、拒絶の発症はない。また、一例でグレードIIの急性GVHDを認めた以外はGVHDを認めていない。移植後、CD3+/CD4+ T細胞は通常のHLA一致移植とほぼ同等の回復が得られているものの、CD3+/CD8+ T細胞の回復は遷延する傾向にあり、T細胞の中でも、Campath-1Hに対する感受性が異なることが示唆された。また、CD3-/CD56+ NK細胞やCD19+ B細胞の回復はHLA一致移植とほぼ同等であった。
一症例において、GVHDの発症と共に残存腫瘍の減少が認められたため、現在、移植後のリンパ球について、ELISPOTアッセイによる腫瘍細胞特異的反応性T細胞の評価を試みている。
