アルツハイマー病の血液診断法の開発に関する研究

Research Abstract

近年、高血圧・糖尿病などの生活習慣病が血管性痴呆のみならず、アルツハイマー病の危険因子でもあることが指摘されている。
一方、アルツハイマー病患者のMRI画像において、無症候性脳梗塞(SBI)はしばしば遭遇する所見であり、誤嚥性肺炎や転倒のリスクを高めるが、その成因については明らかではない。今回の検討では、DSM-IVの診断基準を満たした141名のアルツハイマー病患者の中で、41名(33%)にSBIが認められた。139名の年齢とを性マッチさせた地域在住高齢者を対象とした。SBIを伴うアルツハイマー病では、SBIのないアルツハイマー病に比して有意に年齢が高く、血漿ホモシステイン値が高値であった。年齢を補正した多変量解析では、血漿ホモシステイン値>11.4umol/mlでは、SBIを有するodds ratioは4.2倍(95%C.I:1.7-10.5,p=0.002)に増加していた。しかし、ApoE4遺伝子、高血圧、高コレステロール血症、喫煙習慣などのvascular risk factorsは有意な危険因子ではなかった。また、血漿ホモシステインは、発症年齢、認知機能、脳脊髄液タウ値、脳脊髄液アミロイド値とは相関しなかった。アルツハイマー病に伴うSBIは高ホモシステイン血症を危険因子としてユニークな血管障害と考えられた。血漿ホモシステイン低下療法が、アルツハイマー病における血管障害を予防する上で新しい治療戦略となる可能性が示唆された。
