Research Abstract	本年度の研究では、われわれが実施している癌免疫療法の臨床試験(癌特異的ペプチドワクチン療法第I相試験及び自己活性化リンパ球癌局所投与第II相試験)に登録された癌患者から採取した癌組織の遺伝子発現解析を行うために作成し当大学医学部倫理委員会において承認されたプロトコールを基に、症例の集積を行った。本年度の適格症例12例中、免疫感受性遺伝子解析のための患者説明文書を熟読し組織検体の遺伝子解析に同意した患者は4例で、いずれも自己活性化リンパ球移入療法の臨床試験に登録された症例であった。このうち、臨床試験において免疫療法の効果が得られた症例、即ち癌免疫療法の有効群は2例に認められ、他の2例は不変及び無効群であった。採取保存した活性化リンパ球subsetの変化を解析した結果,培養を継続することによってCD4+CD25+T細胞分画が有意な増加を示し、臨床的にも抗腫瘍効果の減弱を示した症例にCD4+CD25+T細胞の増加が顕著であり,effector T cellの細胞傷害活性に関与していた。また,種々の癌細胞培養株を用いたTRAIL receptor発現実験において、乳癌細胞株や前立腺細胞株、メラノーマ細胞株ではTRAIL receptorの発現を認めたが、食道扁平上皮癌ではその発現が乏しく、プロテアゾームによる効果が期待できないことが判明した。一方,免疫感受性に関連する遺伝子発現解析研究では,いまだ臨床的有効群が2例と少なく、遺伝子解析するまでには至っていないため、今後もさらに検体を蓄積していく予定である。さらに、基礎研究としてCD4+CD25+T細胞増加による免疫感受性低下の防止策についても実験を行った。培養リンパ球を用いたin vitroの実験において、IL-2α鎖受容体に対するモノクローナル抗体を添加すると、CD4+CD25+T細胞の増加が有意に抑制されることが判明した。
