新たな成人悪性リンパ腫疾患単位群の確定を目指した臨床病理学的・分子生物学的解析

Research Abstract

新たな成人悪性リンパ腫疾患単位群として、最近急速に関心を集めつつあるCD5陽性びまん性B大細胞リンパ腫(CD5+DLBCLと略)血管内大細胞型B細胞リンパ腫(IVLと略)、および老人性Epstein-Barr virus (EBV)関連リンパ増殖症(Senile EBV+B-LPDと略)に焦点を当て解析を進めた。
CD5+DLBCLについては2002年に疾患単位a distinct subtypeを形成することを報告するとともに、現在、全国主要施設より123症例を集積した。臨床病理学的検討を継続中であり、病理組織学的亜群giant cell-rich variantの存在とその予後的意義を明らかにした。現在、さらにcomparative genomic hybridization (CGH)法による解析を企図しつつある。同様にIVLについても多施設共同研究として100例以上を集積中である。今後、さらに症例の集積と病態解析を続行する予定である。Senile EBV+B-LPDは、2003年1月に世界に先駆けて新たな疾患単位として報告するに至った。病態は、境界病変(oligoclonal proliferation)から腫瘍性(monoclonal proliferation)まで多岐に亘り、加齢そのものに由来する免疫不全を基盤とする可能性が示唆された。現在、複数の主要施設との共同研究として、びまん性大細胞型B細胞リンパ腫2,000例より100数十例が抽出されようとの見通しの上に検討を進めつつある。これにより、同病変の臨床病態、疾患としての特性をより明らかにする予定である。
