Research Abstract	新たな成人悪性リンパ腫疾患単位群として、最近急速に関心を集めつつあるCD5陽性びまん性B大細胞リンパ腫(CD5+DLBCLと略)血管内大細胞型B細胞リンパ腫(IVLと略)、および老人性Epstein-Barr virus (EBV)関連リンパ増殖症 (Senile EBV-B-LPDと略)に焦点を当て解析を進めた。 CD5+1DLBCLについては2002年に疾患単位a distinct subtypeを形成することを報告するとともに、現在、全国主要施設より123症例を集積した。臨床病態より生物学的特性に至る多面的な検討を継続中であり、病理組織学的亜群giant cell-rich variantの存在とその予後的意義を明らかにした。現在、さらにcomparative genomic hybridization (CGH)法による解析を企図しつつある。さらに、IVLについても多施設共同研究として100例以上を集積中であり、今後の臨床治療研究の礎とすべく詳細な病態解析を施行しつつある。Senile EBV+B-LPDは、2003年1月に世界に先駆けて新たな疾患単位として報告するに至った。病態は、境界病変(oligoclonal proliferation)から腫瘍性(monoclonal proliferation)まで多岐に亘り、加齢そのものに由来する免疫不全を基盤とする可能性が示唆された。現在、複数の主要施設との共同研究として、びまん性大細胞型B細胞リンパ腫2,000例より100例を越える症例を同定した。今後、その臨床病態、予後、治療反応性、および疾患としての生物学的特性をより明らかにすべく解析を進める。世界に類例のない高齢化社会に向かいつつある本邦において、今後、症例の急速な増加が予測され、その病態理解と啓蒙は危急の課題と云えよう。
