Outline of Annual Research Achievements

Heianza Y. et al., J Epidemiol Community Health. 2014; 68 (12): 1189-95.)。世界の糖尿病専門医や研究者らが集まる米国糖尿病学会の学術集会では、「Hot Issues in the Epidemiology of Obesity and Diabetes」のセッションに登壇し、研究成果を口頭で発表した。
健康診査で糖尿病を指摘された後すみやかに通院治療を開始することが、未治療のまま放置することに比べて、どれほど血糖コントロール改善に寄与するかをレセプトと健康診査のデータを用いて大規模疫学的に検討した結果では、特に初診後から継続的に通院治療を受けた場合、より良好な血糖コントロールに寄与することを科学的に証明した。自身が開発した糖尿病リスクスコアの妥当性を検証した結果、開発したリスクスコアは、他の日本人集団でも良好な糖尿病予測精度を有しており、糖尿病発症予備軍の特定に有用であることを認めた。栄養摂取量や身体活動量、心理的要因に関する質問票を用いた大規模調査を行うための、電子データベース化装置を作成し、共同研究者と予備的調査を実施した。

Reason

研究の目的を達成すべく着実に研究実施計画を実行に移した。得られた研究成果を国内・国際学会において発表するとともに、英文原著として多数報告している。本研究での見解は糖尿病予測スクリーニング分野において重要なエビデンスとなることが期待されるとともに、学会ガイドラインの改定のなどにも大きく貢献するはずである。次年度もさらに研究内容が発展し、多くの研究成果が得られることが期待される。

Strategy for Future Research Activity

人間ドックや健診受診者を対象として、栄養・食事・運動・心理的ストレスに関する質問票を用いた大規模な調査を引き続き展開する。
2型糖尿病発症予防のための生活習慣改善介入プログラムを科学的根拠に基づき決定するため、調査で得られたデータ（詳細な生活習慣）が代謝疾患とどのように関連するかを明らかにする必要がある。
一方、生活習慣介入に対する効果や反応には個人差が認められ、遺伝的素因などの対象者背景の違いが、効果反応性に影響を与えると考えられる。そのため、対象者の特徴を考慮した個別化予防戦略が構築できるかについても詳細に検討する必要性があると思われる。
健診受診者に対する糖尿病予防を目的とした介入指導を行い、最適な糖尿病予防介入プログラムの構築に向けた研究を継続して展開する。
