Research Abstract |
We investigated the function and the localization of the left basal temporal language area (LBTA) in patients with intractable left mesial temporal lobe epilepsy as follows. (1) Functions of LBTA by means of electric cortical stimulation Reading, writing auditory comprehension were investigated while LBTA was stimulated. It was concluded that (a) LBTA was involved in visual- and auditory-language processing, at least independently, each dealing with morphological/semantic and phonological information, respectively, (b) kanji words and kana non-words were independently processed in the LBRA, and (c) cognitive processing of kanji words and object/picture partly shared the same process in the LBTA. s, where the patients were requested to read visually presented kanji/ kana words and to name the presented picture/objects silently. As the result, N200 potentials were recognized in response to the silent reading at or very close to LBTA. (3) Neuropsychological analysis in association with left selective hippocampectomy Neuropsychological analysis such as Wechsler Memory Scale-Revised was investigated in patients with intractable left mesial temporal lobe epilepsy after left selective hippocampectomy. After surgery, verbal memory score became higher as compared with before surgery. It could suggest that LBTA could play the significant role in verbal memory function, probably as combined with hippocampus. (4)Clinical methods define the language dominancy in the cerebral hemispheres Wada test with amobarbital is one of the most reliable methods to define the language and memory dominancies in the cerebral hemispheres. We demonstrated that clinically propofol was as useful as amobarbital in Wada test for patients before epilepsy surgery and lesion surgery. Less
