Subclassification of lung cancers without driver mutations

Outline of Annual Research Achievements

１．解析対象例の選定：ドライバー変異のない肺癌90例を抽出した。これらの症例の、組織型、病理病期、予後情報、喫煙歴、アスベスト曝露歴、腫瘍歴、家族歴などを対象症例ファイルとしてまとめた。更に、扁平上皮癌のうち、FGFR1増幅のない扁平上皮癌40例を抽出して、tissue arrayを作製した。
２．ゲノム不安定性の解析：腺癌33例のアレル量データを詳細に再解析し、ゲノム全体のアレル量の変化の程度により、染色体不安定性の程度を、軽度、中等度、高度の3段階に分け、喫煙量との相関を検討した。その結果、LOH頻度と喫煙量とが相関することが判明した。小細胞癌については、予後と相関する4種のmiRNAを抽出した。
３．メチル化異常：20例の小細胞癌について、ゲノムワイドメチル化解析のデータの取得が完了したので、部位の解析、パスウェイ解析を行った。その結果、末梢型小細胞癌では、TTF-1パスウェイが活性化していた。
４．非コードRNA の検索：腺癌については、非コードRNA の発現パターンと、臨床病理情報、ゲノム不安定データ、その他の因子との相関を継続して検討中である。
５．PD-L1の発現と予後:肺腺癌において、PD-L1の発現を検索し、予後との相関を検討した。その結果、一般にPD-L1高発現の症例は予後不良であった。この傾向は、喫煙者でのみ有意であり、非喫煙者では、予後の差は無かった。

Reason

ほぼ予定通りに進捗している。採択以降、免疫チェックポイント阻害薬が注目されるようになったので、ゲノム不安定性・メチル化の費用をPD-L1解析に向けている。

Strategy for Future Research Activity

ほぼ当初の予定通り推進する予定であるが、免疫チェックポイント阻害薬のバイオマーカーになると推定されるPD-L1の発現が、ドライバー変異のない癌で高いことが判明したので、その方向の研究を目的に加え、ゲノム不安定性の解析・メチル化解析のうち、費用のかかるものについては、実施を再検討している。
