Outline of Annual Research Achievements

1) てんかん焦点における広域周波数帯域脳波解析を用いた低周波活動の記録解析:多施設からの難治部分てんかん患者を対象に共通の方法を用いて広域周波数帯域皮質脳波解析を行った結果、てんかん発作発現に伴う低周波活動である発作時DC電位(グリアの関与が示唆)が、高周波律動(HFO:ニューロンの高頻度発火、介在ニューロンの活動を反映)に対して早期に出現することを明らかにした(国内学会発表)。既報告に合致した結果であり、発作発現時にグリアとニューロンが密接に関連しつつ異なる役割を担う可能性が示唆された。多施設における解析の過程で得られた知見をもとに広域周波数帯域脳波の記録・解析標準化案を作成した(論文発表)。
頭蓋内電極留置期間中に、頭皮上に少数の電極を貼付して睡眠段階を同定し得た患者において、覚醒・睡眠段階で分類した皮質脳波を用いた異周波数間結合の解析を継続した。高周波数帯域の瞬時振幅と低周波数帯域の瞬時位相の相関解析(phase-amplitude coupling)を行い、徐波睡眠期に低周波と高周波数帯域の異周波数間結合が強まること、脳葉、てんかん原性の有無によって結合の程度、睡眠段階による結合動態が異なることを明らかにした(国際学会発表、論文準備中)。
2) 機能的MRI(fMRI)を用いた記録解析:視床下部過誤腫におけるてんかん患者を対象にてんかん外科手術前後の安静時fMRIの記録・解析を行った。てんかん焦点である視床下部過誤腫と大脳皮質間の機能的結合は術後に減弱する一方で、他の機能的ネットワークについては有意な相関を示すものが術後に増加しており、手術によるてんかん発作の抑制、てんかん性脳症の改善との関連性が推察された(国内学会発表)。
