Outline of Final Research Achievements

About 80% of rapid eye movement sleep disorder (RBD) is known to progress to alpha-synucleinopathy such as Parkinson's disease or Lewy body disease. In this study, comorbidity rate of RBD and alpha-synucleinopathy related physiological/neuropsychological findings in narcolepsy were investigated. For idiopathic RBD patients, almost half of patients showed pareidolia response similarly to Lewy body disease. However, narcolepsy patients and RBD comorbid with narcolepsy did not show these alpha-synucleinopathy related physiological/neuropsychological findings. These results indicated different pathology between RBD and RBD comorbid with narcolepsy.

Academic Significance and Societal Importance of the Research Achievements

ナルコレプシーには,睡眠時随伴症や睡眠関連運動障害が合併しやすいことが報告されていたが,本邦においては検討されてこなかった.本研究において,ナルコレプシータイプ1にはレム睡眠行動障害が1.9%合併することが明らかになった.同時に,将来的に神経変性疾患へ移行するレム睡眠行動障害は,ナルコレプシーに合併した場合でも特発性レム睡眠行動障害同様に神経変性疾患発展リスクを有するのか,また,レム睡眠行動障害同様に嗅覚機能や認知機能が低下するといわれていたナルコレプシー患者の将来的なリスクはいかなるものかを検討した結果,特発性レム睡眠行動障害とは病態を異にし,同リスクは認められないことが明らかになった.
