Outline of Annual Research Achievements

本研究は、2002年より継続しているHTLV-1感染者追跡コホート(JSPFAD)の維持と、蓄積された経時的疫学情報と検体を用いて、マルチカラーFACS、ウイルスマーカー、バイオマーカー、分子マーカー情報などを組み合わせたATL発症高危険群の指標と成り得る発症予測スコアリングを構築し、ATL発症予防や早期治療介入に資するエビデンスを創出する事を目的としている。平成28年度の年間新規登録者は182例、新旧登録者からの提供検体数は1,100であり、コホート全体の総登録数は3,500例、総検体数は12,055となった。本年度は、コホートの現状とバイオマテリアルバンクを利用して行われたこれまでの研究成果を取り纏めて関連施設へ報告し、研究担当者別に下記の３項目について研究を進めた。1) 疫学データベースの再整備：コホート研究の質の担保のために、関連施設データのモニタリングとクリーニングを再開した。特に欠損値の多かったsIL2R値の補充対策に取り組み、本年度内に約7割の欠損値を充填した。2)HAS-Flow法によるATL移行細胞の経時的測定系のコホートへの応用： HTLV-1キャリアコホートにおいて、HAS-Flow法のデータがATL発症細胞の経時的マーカーと成り得るかどうかを検討するために、本年度はまず、JSPFADサンプルを用いてHAS-Flow解析を行うためのセットアップを行い、その後、19例について経時的測定の条件設定などを検討した。3)リスクインディケータとしてのバイオマーカー探索研究： HTLV-1キャリア体内の不死化感染細胞や腫瘍化細胞における分子異常の特徴を明らかにする作業をJSPFADの検体を用いて進めるとともに、整理集約して発症リスクおよびプログレッションのバイオマーカー探索を進めた。

Reason

疫学データベースの再整備においてsIL2R値の補充を重点的におこなったために、予定していた施設モニタリングの頻度が少なくなったが、その他の研究行程は概ね本年度中の目標を達成していると判断した。

Strategy for Future Research Activity

平成29年度は、前年度から継続して、相互に連携を取りながら、研究担当者別の研究をさらに進める予定である。具体的には、
1) 疫学コホートの維持・管理と並行して関連施設データのモニタリングとクリーニングを頻回に行い、sIL2R値以外のデータベースの再整備を継続する。
2) コホート検体を用いて、HAS-Flow法によるATL移行細胞の経時的測定系の確立を目指した作業を進め、プロウイルスロード・sIL2Rと同様に、疫学研究と連動した経時的な測定体制を検討する。
3) コホート検体より抽出したバイオマテリアルを用いて、キャリア体内の不死化感染細胞や腫瘍化細胞における分子異常の特徴を明らかにする作業を進める。特にHAS-Flow法による結果と連動して、個体内でのプログレッションを明らかにするマーカー候補を絞り込み、発症リスクおよびプログレッションのバイオマーカー探索を継続する。
