| Outline of Annual Research Achievements |
術前化学療法を行った後に外科的切除が行われた大腸癌肝転移47例について，造影CTによる形態学的評価(morphologic appearance)：1)Overall attenuation(全体的な特性)，2)Tumor-liver interface(肝転移巣と肝実質の境界)，3)Peripheral rim enhancement(辺縁の造影効果)に対し，種々の組織学的所見がどのように反映されているかを検討した．1)のCT所見の変化は，粘液湖の増加，腫瘍壊死(usual necrosis, UN)から梗塞様壊死(infarct-like necrosis, ILN)への壊死の性質の変化により説明された．2)のCT所見の変化は，ILNの増加，腫瘍細胞の残存率を反映していた．3)の所見の変化は，UNの減少，腫瘍細胞の残存率（特に完全奏功したかどうか)が関連していた．なお，組織学的評価にあたってはUNは腫瘍が残存した領域，ILNは腫瘍が消失した領域として判定することが重要であった．VEGFを阻害する分子標的治療薬であるBevacizumab (BV)を使用した治療群は使用していない治療群(non-BV)と比較して，speckled necrosis(まだら壊死)よりもcentral necrosis(中心性壊死)を示す傾向がみられたが画像所見には反映されていなかった．適切な画像所見の評価は治療方針に重要である．本研究から，造影CTの所見がどのような組織所見によって説明し得るかが明らかになり，CTにおける術前化学療法の効果判定を生じている組織変化を予想しながら行うことが可能となった．

| Strategy for Future Research Activity |
がんの微小環境について大腸癌の原発巣と肝転移巣についての解析を進めている．また，本研究を行うことで手技が確立したepithelial-mesenchymal transitionの免疫組織化学を用いて，様々な癌腫(膵癌，皮膚癌など）における腫瘍発生・進展のメカニズムが明らかになりつつある．特に原発と転移の違い，組織型の相違ががん間質にどのように関連しているのかについて検討をすすめる．
