Research Abstract |
Purpose) The purpose of this investigation is to develop a method to analyze the morphological and functional abnormality of the soft and hard tissue of the patients with temporomandibular joint (TMJ) arthrosis more correctly than 1. 5T MR ultilizing a new dual surface coil for 3. OT MR. Materials and Methods) Nine asymptomatic and symptomatic volunteers were examined by 3T MR with dual surface coil under the permission of the ethics committee of Tokushima University Hospital. 1.New 3-inch Dual Surface Coil for 3. OT MR imaging of TMJ: Using 3-inch Dual Surface Coil and upgraded imaging software optimized imaging method (ex. imaging sequences, positioning of imaging layers ) to obtain clear image of TMJ were investigated. 2.Development of a method to analyze morphological and functional abnormality of TMJ: Simple imaging and analyzing methods of morphology and dynamic movement of TMJ three-dimensionally were investigated. : Imaging of TMJ by sections perpendicular to the long axis of condylar head was suitable for the imaging of morphological changes of soft tissue of TMJ. For the purpose of analyzing dynamic movement of TMJ imaging of TMJ by sections parallel to sagittal plane was possible. 2. Development of a method to analyze morphological and functional abnormality of TMJ: 1)Morphological analysis: Morphological analyze of TMJ by MPR (multiplaner reconstruction) and 3D imaging by many kinds of viewing software could be performed. More clear image could be obtained. Measurement became more easy and correct. 2)Functional analysis: 3D Pseudo-Cinematographic imaging of anticular condyle and disc during open-close mandubular movement became possible. Semi-automatic image analysis using simple threshold method could not separate compartments of TMJ. Free-handed tracing of these compartments wastes huge times and lacked accuracy on alignment of image layers. We used semi-automatic edge detecting method connecting points on the contour of the components by spline curve. This method minimized analyzing time and improved accuracy. For the application to the clinics an analyzing method which can diagnose the dynamic movement of TMJ is necessary utilizing suitable computer-aided diagnostic (CAD) image processing considering characteristics of 3.0T MR image of TMJ.
