| Outline of Annual Research Achievements |
前年度に示した、HER2陽性乳癌の治療効果に関して、本年度Androgen receptor (AR) 発現との関連を詳細に検討した。
HER2陽性乳癌は、周術期化学療法と抗HER2療法を併用することが推奨されているが、治療効果はHER2陽性乳癌サブグループ間で一定ではない。
【対象と方法】術前補助療法が施行されたHER2陽性浸潤癌60例を対象とし、ホルモン受容体陽性群（LH群）とホルモン受容体陰性群（NLH群）に分類した。各群に対して術前針生検検体におけるARの免疫組織染色による発現を評価し、組織学的治療効果はgrade 2b以上をnear 病理学的完全寛解 (pCR) し、ARの発現と治療効果との関連を検討した。
【結果】症例の内訳は、LH群が35例 (58.3%)、NLH群は25例(41.7%)であり、それぞれ30例、14例がAR+。pCRは25例（41.7%）[ LH群: 11例（31.4%）、NLH群: 14例（56%）]、near pCRは30例（50%）[LH群: 15例（42.9%）、NLH群: 15例（60%)]。AR+のpCRは19例 (63.3%）（p<0.001）、near pCRは23例 (76.7%)（p<0.001）であり、ともにAR-と比較して有意に高かった。サブグループ間では、LH群におけるAR+のpCRは9例（56.3%）(p<0.01)、near pCRは12例 (80%) (p<0.01)であり、ともにAR-と比較して有意に高かった。一方、NLH群のAR+のpCRは10例（71.4%）(p=0.08)であり有意差はないものの、near pCRは11例 (78.6%) (p=0.03)であり、AR-と比較して有意に高かった。
【結語】AR発現はHER2陽性乳癌において、LH群、NLH群ともに治療効果予測因子として有用である可能性が示唆された。
