Outline of Final Research Achievements

Depressive mixed state (DMX) in patients with major depressive episode was examined using the 12-item quantification scale for DMX (DMX-12) developed by us. Main DMX symptoms were hypersensitivity, racing/crowded thought, distractibility, overreaction, and inner tension. Severely depressed younger subjects with potential bipolarity are more likely to develop DMX, especially with disruptive emotion/behavior symptoms.
Selected 8 symptoms from the DMX-12 may be helpful in screening of DMX with considerable severity since it well differentiates individuals with mixed depression or mixed features during major depressive episode from non-mixed patients, with the same cut-off and excellent sensitivity/negative predictive value.

Academic Significance and Societal Importance of the Research Achievements

過少診断されやすい抑うつ性混合状態が、我々が開発したDMX-12により重症度が数値化され、一定の重症度を持った抑うつ性混合状態のスクリーニングが可能となった点で、臨床的な診断意義は大きい。
本スクリーニングの使用により、陰性例では十分な抗うつ薬治療を安全に進め、陽性例では抑うつ性混合状態の診断的な面接に進み、抗うつ薬を回避した薬物選択(気分安定薬や非定型抗精神病薬の導入)を合理的に選択することが可能となり、治療の質の向上や薬効判定にも寄与する点で社会的意義も大きい。
