Outline of Annual Research Achievements

慢性腎臓病(CKD)の主たる原因疾患である糖尿病、高血圧、加齢では、共通して血管内皮障害を伴う。糸球体、尿細管いずれにおいても内皮と上皮(ポドサイト、尿細管上皮)は近接しており、機能連関が存在する。内皮障害はどのような機序で内皮/上皮連関を破綻させ、CKDの共通進展機序である「炎症」と「線維化」を惹起するのか、内皮障害と上皮障害を結ぶ機序は何か、活性酸素種、一酸化窒素(NO)及び両者の均衡変化、NLRP3 Inflammasome、Wnt/β-catenin経路、Keap1/Nrf2経路、ミトコンドリア障害の役割に焦点をあてて解析した。内皮特異的NADPH oxidaseマウスに糖尿病モデルを作成し、内皮に酸化ストレスを負荷したところ、アルブミン尿出現前の早期から、内皮障害だけでなく上皮障害(内皮/上皮病態連関)を認めた。また尿細管間質病変における内皮障害、内皮/上皮病態連関の役割を解析した。eNOS KO-UUOにおいて間質炎症・線維化が増悪することを見いだした。さらに内皮機能の保持による腎保護効果の可能性を示した。
慢性腎臓病(CKD)は末期腎不全のみならず、心血管病、認知症発症のリスク因子でもある。超高齢化社会を迎え、一層の増加が危惧される。CKDの進展と合併症との連関機序を解明し、有効な予防・治療法を開発することが急がれる。CKDの病態形成に内皮障害が関わることが想定されながら、研究技術上の障壁により解明が遅れていた。私共は新規研究技術の開発にとり組んだ。その結果、生活習慣病、加齢によるCKDでは内皮障害がその発症に関与し、活性酸素種と一酸化窒素が病態形成のメディエーターとなる。内皮障害が内皮/上皮連関を破綻させ、腎障害の2大病態である「炎症」と「線維化」を惹起させ、CKDを重症化させるのかを解明した。
