Outline of Final Research Achievements

This study focused on two main areas: expanding web-based support to help people who have difficulty attending health-related events due to work, and organizing engaging health classes not limited to diabetes. It also explored ways to strengthen collaboration among community members, Certified Diabetes Educators (CDEs), support groups, and facilities, while considering joint programs with local governments, universities, and welfare organizations. The findings suggest that decision-making processes such as Advance Care Planning (ACP), where individuals reflect on their future based on their own values and judgments, can help deepen mutual self-assessment. To support diverse values and perspectives, CDEs need continuous development of professional knowledge and sensitivity. The importance of providing opportunities for dialogue and interaction to enhance self-care abilities was also highlighted.

Academic Significance and Societal Importance of the Research Achievements
