| Research Abstract |
The discharge nursing program developed through this research was consisted of five elements: approach for patients, approach for professional team members, approach for family/people who close to patients, approach for myself (nurse), and approach for organization/system. Through applying the program in clinical practice, it is hoped that community care for long term inpatients will be promoted.
