うつ病における脳の拡散テンソル異常に関する研究

Research Abstract

(1)難治性うつ病患者13名(男性9名、女性4名)、非難治性うつ病9例(男性5名、女性4名)、および健常者22例(男性14名、女性8名)において、1.5T装置で撮像した拡散テンソルのグループ解析を行った。その結果、one way ANOVAおよびpost-hoc t-testで、難治性うつ病患者群では、健常者と比べて、両側前頭葉白質の一部に有意なFA(fractional anisotropy)の低下が認められた。非難治性うつ病と難治性うつ病、非難治性うつ病と健常者との間には、有意なFAの差異は認めなかった。いずれの群間でもMD(mean diffusivity)には有意差を認めなかった。難治性うつ病では、limbic-striatal-pallidal-thalamic(LCSPT)および/またはlimbic-thalamo-cortical circuitに異常が生じている可能性があると推測された。難治性うつ病における異常FA部分のFA値と、うつ病の臨床パラメータ(GAF scale、HDRS、MADRS)には有意な相関は認められなかった。しかしながら、患者数がやや少ないため、さらに患者数を増やして検討を続けることとした。
(2)平成21年度に、うつ病が疑われ、かつ、未治療の患者17名において、散テンソル撮像を行った。治療後に、計12回の経過観察の拡散テンソル撮像を行った。平成20年度以前も含めると、25名、延べ55回の拡散テンソル撮像を行った。現在、これら患者が、うつ病であるかどうかの確認作業中である。今後、うつ病と確定した患者に関して、健常者との拡散テンソルのグループ解析を行う予定である。
