うつ病における脳の拡散テンソル異常に関する研究

Research Abstract

(1)難治性うつ病患者13名、非難治性うつ病12例、および健常者22例において、1.5T装置で撮像した拡散テンソルのグループ解析を行った。その結果、voxel-based analysis(one way ANOVAおよびpost-hoc t-test)で、難治性うつ病患者群では、健常者と比べて、両側前頭葉白質の一部に有意なFA(fractional anisotropy)の低下が認められた。非難治性うつ病と難治性うつ病、非難治性うつ病と健常者との間には、有意なFAの差異は認めなかった。また、うつ病(難治性および非難治性)では、健常者と比べて、両側前頭葉白質の一部にMD(mean diffusivity)上昇を認めた。難治性うつ病では、limbic-striatal-pallidal-thalamicおよび/またはlimbic-thalamo-cortical circuitに異常が生じている可能性があると推測された。難治性うつ病における異常FA部分のFA値と、うつ病の臨床パラメータ(GAF scale、HDRS、MADRS)には有意な相関は認められなかった。
(2)平成20年度以降に拡散テンソル撮像が行われた未治療(treatment-naive)のうつ病患者22名を、健常者22例と比較した。voxel-based analysis(one way ANOVAおよびpost-hoc t-test)で、患者群において、健常者と比べてFAやMDが有意に異なる部位は認められなかった。これらの患者では、FA低下部位が、患者毎に異なっている可能性が示唆された。今後、これらのうつ病患者の治療後経過観察を行い、拡散テンソル異常との関係の解析をさらに進める予定である。
