Outline of Annual Research Achievements

本研究の目的は、加齢に伴う聴覚障害（加齢性難聴）が歩行機能と認知機能の関連に対し、どのような影響を与えるのか、その媒介効果と高齢期の転倒発生への影響・機序を明らかにすることである。本年度は加齢性難聴が脳機能に与える影響を中心に検討した。
研究には164名の地域在住高齢者が参加した。オージメータを用いて聴力検査を行い500Hzから4000Hzの聴力閾値を左右耳で測定し、平均聴力レベルを算出した。左右の耳のうち、聴力が良い方の耳の平均聴力レベルが41dB以上の者を加齢性難聴者（正確には中等度以上の加齢性難聴者）と定義した。また参加者にはMRI検査に加え、FDG-PET（positron emission tomography with 18F-fluorodeoxyglucos：血中にブドウ糖化合物であるFDGを投与し、FDGの脳取り込み量を測定することにより脳糖代謝量、すなわち神経活動を推定する方法）検査を行い、脳の構造と神経活動を測定した。参加者のうち中等度以上の難聴者は29名（17.7％）であった。これら難聴者と非難聴者の脳構造を比較したところ、有意な差は認められなかった。しかしながら、非難聴者に比べて難聴者では聴覚皮質を含む両側上側頭回における糖代謝が有意に低いことが明らかとなった。本研究の結果から、高齢期の難聴が感覚器のみの問題ではなく、中枢の機能も関与する問題であることが示唆された。
また本年度は並行して、東京都健康長寿医療センター耳鼻咽喉科・補聴器外来を受診している高齢者を対象とした補聴器装着に伴う歩行機能と認知機能改善効果に関する研究の追跡調査も行っており、最終年度中には全ての研究参加者の追跡を終了する予定である。

コロナ感染に伴い、臨床研究はやや遅れているが、全体として研究計画は順調に進んでいる。

Strategy for Future Research Activity

来年度は臨床研究における追跡調査を行うとともに、実験室研究を遂行する予定である。これらの準備は済ませており、問題なく実施できる予定である。
