Outline of Final Research Achievements |
In older patients undergoing cardiac surgery, physical function is a critical determinant of postoperative outcomes. HMB supplementation has been shown to promote muscle protein anabolism and inhibit catabolism, thereby preventing muscle weakness. However, its efficacy in older patients undergoing cardiac surgery remains unknown. This study aimed to examine the effects of preoperative HMB supplementation on postoperative physical function and complications in this population. Preoperative HMB supplementation in older adults undergoing cardiac surgery resulted in significant improvements in postoperative exercise capacity and physical function, along with a reduction in the length of hospital stay, without affecting muscle mass. Therefore, preoperative HMB supplementation could be a valuable prehabilitation strategy, particularly for patients with diminished physical function.

高齢化が進行している本邦において、高侵襲手術である心臓手術後の合併症やADL低下が非常に問題となっている。その問題点に対して術前の栄養介入、特にHMBを用いた介入は大きな解決策となる可能性がある。術前のHMB介入により、術後のADL、QOL低下を抑制でき、術前の標準的な栄養療法の提示、また周術期有効なリハビリテーションの確立の一助となった。また、身体機能低下により元来手術適応とならなかった患者にも手術適応の道が開ける可能性がある。
