Research Abstract

成羊を用い、臨床的に行なわれている前十字靱帯再建術を模倣するモデルを開発し、その自然経過(一年)を生体力学的、超微細構造学的および分子生物学的に明らかにした。
成羊(サフォーク種)を用い臨床ですでに開発している靱帯再建用手術器具を用いて右膝の半腱様筋腱を採取し、移植用材料を作製した。前十字靱帯の正常付着部に移植用の骨孔を作製して移植材料を移植し無細胞マトリクスをin vivoで作製した。術後0週、12週、24週、および52週で屠殺し、組織学的観察および万能引張試験機を用いた生体力学的評価を行った。力学的試験は、膝関節を摘出後、万能試験機を用い一定の前後方向荷重下に膝前後動揺性を計測した。次に大腿骨-前十字靱帯(移植腱)-脛骨複合体を摘出し、Video-dimension analyserを組み込んだ生体軟組織材料試験システムを用いて37℃生理食塩水中での力学的パラメータの測定を行い、断面積、最大破断荷重、線形剛性、破断伸び、引っ張り強度、弾性率、破断歪みを測定した。
その結果、組織学的には術後12週において無細胞領域が観察されたが、術後52週では線維芽細胞様細胞が移植腱の深層まで浸潤し規則的なコラーゲン配列を認めた。力学的評価では、術後24週までは全てのパラメータが正常群よりも低値であったが、術後52週において膝前後動揺性は正常群より有意に大きく、最大破断荷重は正常群より低値であった(p<0.05)。しかし、線形剛性は正常群と有意差がなかった。引っ張り強度および弾性率に関する比較も同様な結果であった。
