メタボリックシンドロームにおける鉄代謝異常の機序とそれに基づく新規治療法の開発

Research Abstract

1.平成21・22年度には鉄制限がメタボリックシンドロームモデルであるOLETFの糖尿病性腎症発症を予防した。平成23年度には、糖尿病性早期腎症が発症した後に、鉄制限が治療効果を有するかどうかを検討した。OLETFは30週齢ころから微量アルブミン尿が出現する。さらに40週齢まで飼育し、そこで次の4群に分けた。(1)正鉄食群(通常の食事)(2)軽度鉄制限食群(添加Fe量を通常の3分の1にする)(3)中等度鉄制限食群(添加Fe量を通常の10分の1にする)(4)高度鉄制限食群(Feを添加しない食事)。そして4週間おきに体重測定、血圧測定、蓄尿、採血を行い、52週齢ですべて安楽死させ血液と腎臓のサンプルを得た。その結果、予想に反して、蛋白尿は各群間で全く差がなく、腎障害の程度も差がなかった。その原因として、高度鉄制限食群でさえ、正鉄食群に比べてヘモグロビンが低下しなかったためと考えられる。メタボリックシンドロームが進行した段階ではかなりの鉄が貯蔵されていて、8週間の鉄制限では貯蔵鉄を減少させることが出来なかったためかもしれない。さらに長期間の鉄制限を行うことが、鉄制限の糖尿病性早期腎症に対する治療効果を検討するためには必要である。
2.臨床応用の可能性を探るため、健常成人男性における瀉血が酸化ストレスに及ぼす影響を検討した。対象は健常男性において、献血の1週間前、1週間後、4週間後に早朝空腹時に血圧、脈拍、血液検査(CBC、血清鉄、フェリチン、TIBC、酸化LDLなど)、尿中8-hydroxy-2'-deoxyguanosine(8-OHdG)、尿中クレアチニンを測定した。献血前後の酸化ストレス(酸化LDL、尿中8-OHdG/Cr比)の変化度とHb,フェリチンの変化度に相関はなかった。若年健常者ではもともと酸化ストレスが低く、鉄を減少させてもあまり変わらなかった可能性が考えられた。
