| Outline of Final Research Achievements |
In this study, we evaluated the effects of Kshara Sutra (KS), a therapeutic thread for anal fistula, in a rat model of fistula, and found that KS treatment resulted in a slight increase in the expression of inflammatory markers IL-6 and TNF-α, and a loss or significant thinning of the epithelial layer of the fistula tract. In addition, KS showed no antimicrobial activity at low concentrations against anal fistula pathogens.The loss or inhibition of the formation of the epithelial layer of the fistula tract by KS may contribute to a shorter treatment period.

| Academic Significance and Societal Importance of the Research Achievements |
痔瘻は患者QOLを低下させる重大な病気である。痔瘻に対し、KSはこれまでに患者負担の少ない治療法として多くの臨床報告がなされている一方で、基礎研究によるエビデンスが不足しており、優れた治療法としての可能性を持ちながらも普及には至っていない。本研究によりKSによって瘻管上皮層が欠損し、炎症が誘発されることが明らかとなったため、今後はこれらをターゲットにした更なるメカニズムの探求や、薬の開発へと応用が期待される。
