Research Abstract

関節リウマチ(RA)にしばしば伴う間質性肺病変(ILD)は関節外病変の一つであり、予後に大きな影響を及ぼす。RA治療ではメトトレキサート(MTX)と生物学的製剤の普及により、薬剤誘発性ILDが生命予後を左右する重篤な有害事象として問題となっている。RAに合併したILDとRA患者における薬剤誘発性ILD発症とに、ヒト白血球抗原(HLA)が関連するかどうかを明らかにすることを目的とする。ILDの合併と薬剤誘発性ILDの既往が明らかなRA症例について、HLA関連解析を行った。HLA-DRB1*04, shared epitope (SE), DQB1*04はILD発症のリスクと負の関連を示し、DRB1*16, DR2血清型 (DRB1*15, *16), DQB1*06はILD発症と正の関連を示した。MTX単独投与または生物製剤との併用中に発症した薬剤誘発性ILDは、HLA-A*31:01と関連した。
