Multifaced imaging analysis in patients with intractable epilepsy: a study for surgical treatment approach

Outline of Annual Research Achievements

側頭葉てんかん症例にてMR高分解T2強調画像と３DT1強調画像を用いたsubfieldの自動抽出ソフト（ASHS）と、従来のfree surferと比べて、subfieldの識別能の評価を行った。対象は45例の正常例、50例の側頭葉てんかん患者（内25例の海馬硬化症と25例の非海馬硬化症）にて、ソフトは高分解能free surferの新・旧バージョンを用いた。ASHSと新free surferでは、患側のC1.C4,歯状核の高いZスコアーを示したが、非海馬硬化症ではいずれのsubfieldも体積の減少は認めず、今後この手法は海馬のsubfieldの評価に有用と考えられた。
また側頭葉てんかんにて最近偏桃体腫大を伴う症例が報告されるようになったが、まだそのetiologyに関してはコンセンサスを得ていない。今回我々は海馬硬化症と偏桃体腫大を伴う側頭葉てんかん症例、それぞれ34例と17例にて、大脳の白質の萎縮やFA値を比較したところ、前者では著名な白質のvolumeの低下とAF値の低下を認め、両者のetiologyは違うことが示唆された。
また特発性全般性てんかん患者において血流の比較とグラフ解析を行った。ASLの脳血流データをもとに、19例の患者と19例のコントロールにて比較した。患者群においては視床、上部中脳、左小脳に血流の低下が認めまた。またグラフ解析では、両側の側頭後頭葉、外側前頭葉に脆弱性を認め、これらの手法は特発性全般性てんかんの理解に役立つものと思われた。
