膵癌幹細胞の病理学的性質を規定するマイクロRNA機構の解明

Outline of Annual Research Achievements

本邦では、年間約3万人の膵癌の罹患者がおり近年増加傾向である。しかも年間罹患数と死亡数が等しく難治癌の代表であり、安全で有効な治療法の確立が強く望まれる。申請者はこれまで642人の膵管内乳頭粘液性腫瘍患者の前向き観察研究を行い、同疾患が膵癌の高危険群であることを証明し報告してきた。しかし従来の臨床的手法では膵発癌の超高危険群の同定、早期発見が困難であることから、大部分の膵癌は診断時には、切除不能であり、平均生存期間が約6カ月と極めて予後不良である)。
膵癌の早期発見のため、膵管内乳頭粘液性腫瘍の患者から、嚢胞液または膵液の採取を行った。年齢中央値は64歳（範囲：54-78歳）、男性4例女性1例、主膵管型2例分枝型3例、鉤部2例頭部3例。抽出したDNA量は、360-7960ng。コントロールとしては、患者さんの末梢血の白血球からDNAを抽出し、それらを用いた。IPMNの悪性度は、IPMN low grade(LG) 2例、IPMN high grade (HG) 2例、IPMN with invasive carcinoma (IC) 1例。観察期間中央値は9カ月。IPMA low gradeの2例は、経過観察中、IPMN high gradeの1例は外科的切除を行い、もう1例は今後外科的切除の予定。IPMN with invasive carcinomaの症例は、化学療法を現在も行っている。
