| Outline of Annual Research Achievements |
本研究は、高齢者の活動の促進によって認知症予防を目指すポピュレーション・アプローチのシステムの構築と効果検証を行うため、自治体、企業、住民と共同して研究を推進した。対象とした自治体は、愛知県高浜市（人口46,000名、高齢化率17％）である。平成29年度は、高齢者機能健診受診者4,122（女性：2,319名、男性：1,803名、平均年齢：71.4歳）名の活動量データ収集を、高浜市内の98か所の健康自生地に活動量計データ収集端末を設置して行った。なお、健康自生地とは高浜市が認定した高齢者が集うことのできる場所やサービスである。1年間で10か月以上の活動データが収集できた群（活動実施群）と一度も収集できなかった群（活動非実施群）をプロペンシティスコアマッチングにて属性を調整した集団を作り（両群ともに1106名）、1年間の要介護認定発生について比較をした。対象者は2212名（女性：1225名、男性：987名、平均年齢：71.3歳）であった。両群に年齢、性別、現病歴（心疾患、呼吸器疾患、脳卒中、うつ病、パーキンソン病、高血圧、糖尿病、脂質異常症、変形性関節症）、服薬数、日常生活動作、歩行速度、mini-mental state examination、geriatric depression scaleにおける有意差は認められなかった。1年間の追跡期間中に24名（1.1％）の対象者が要介護認定を受けた。Cox比例ハザードモデルにより活動の実施と要介護認定の発生との関係を調べた結果、活動非実施群に比較して実施群では要介護発生のハザード比が0.14（95％信頼区間：0.04-0.48）であった。以上の結果から、日常的に活動を実施することは要介護の発生を回避するための手段になり得ると考えられた。
