Outline of Final Research Achievements

With the social theme of development of Community Integrated Care Service System, this study aimed to build a support cooperation modeling tool as educational content toward effective inter-professional education for community care participants. Through the results of the investigations on practices in Netherlands and Japan, the importance of positioning self- and mutual help in community care cooperation and providing education for self- and mutual help was clarified. The modeling tool for IPE was created on the basis of ERD (entity-relationship diagram) idea with the data organized from service record documentations of home-based care service with living support. The tool was tentatively checked by pro-participants including care takers, care manager, nurses and registered dietitians to find its effectiveness and task. Although further development is necessary, it was found as effective for education for community care/support cooperation including self- and mutual help.
