Outline of Annual Research Achievements

難治てんかんに対する迷走神経刺激療法（ＶＮＳ）の発作抑制有効率は、50％程度にとどまり、どの様な患者に有効か、ＶＮＳ用ジェネレータ埋め込みの術前に予測する方法は明かにされていない。本研究では、これら神経ネットワークの定量的解析や、脳波の特徴と治療成績を比較検討し、さらに、低周波経皮刺激装置を用いた脳の賦活を応用し、ＶＮＳの作用機序を検討し、レスポンダーを規定する因子（バイオマーカー）を評価し、より効率の良いＶＮＳ手術適応条件を見いだすことが目的である。
当院において、難治てんかんへＶＮＳ施行した45例のうち、フォローアップが可能であった35例を後方視的に解析した。（男性23例、女性12例、年齢7～60歳、平均年齢26歳。）24例で発作頻度が半分以下に減少し、3例で抗てんかん薬の減量に至った。また3例で鬱症状が緩和された。当院におけるＶＮＳの効果が良好であることが確認され、刺激による精神症状の緩和も得られた。昨年度は数名のVNS施行患者において４週間の観察期間をおき、ベースラインの脳波、安静時FDG-PETを計測した。FDG-PETは、３D-SSP（three-dimensional stereotactic surface projections）を用いて、提体者あるいは交代者として統計的に優位とナル部分を定量的に確認した。さらに紫班の低周波刺激器の刺激パラメータを計測し、患者への施行を準備している。
