Outline of Final Research Achievements |
To reveal the relation between chronic pain in geriatric patients and changes in gene expression or epigenetics at peripheral blood cells, clinical investigations were performed in patients underwent surgery or suffered with chronic back pain. Peripheral nerve injury, which is one of the causes of persistent post-operative pain and chronic back pain, likely increases brain-derived neurotrophic factor (BDNF) in the central nervous system under pro-inflammatory signals, and decreases under anti-inflammatory signals. This study showed that epigenetic changes in BDNF genes in the peripheral blood cells cause the decrease in the serum concentrations of BDNF with anti-inflammatory state in patients suffering from chronic back pain and that the decrease of serum BDNF correlates with the increase in the number of pain symptoms. BDNF at peripheral blood cells might be a key factor for a transition from acute post-operative pain to persistent post-operative pain.
