子宮体癌・卵巣癌においてアポトーシスを誘導する新規分子標的治療法の探索

Outline of Annual Research Achievements

【目的】子宮体癌の局所的な治療において放射線照射が広く行われているが、放射線のみでは感受性が低いことが課題となっている。そこで子宮体癌細胞において、放射線照射によって活性化されるシグナル伝達経路を検討し、放射線感受性を増強する新規治療法を見出すことを研究目的とした。
【方法】子宮体部細胞株8種類を用いて、放射線照射（2Gy～10Gy ）を行い、MAPK経路、PI3K/mTOR経路の活性をImmunoblotting法にて解析した。Clonogenic assayを用い、TP53変異と放射線感受性の相関を調べるとともに、照射後に活性化される経路の阻害が放射線感受性を増強するかを検討した。また、siRNAを用いて標的分子のノックダウンにより感受性増強に関わる因子を調べた。
【成績】子宮体癌細胞株8種類に放射線照射を行ったところ、TP53変異陽性5株では、変異陰性3株に比べ、感受性が有意に低かった（p＜0.05）。また、TP53変異によらず、MAPK経路（ERKのリン酸化）、PI3K/mTOR経路（AKTのリン酸化）が活性化されていた。併用実験では、MAPK経路阻害剤は有意な放射線増強作用を示さなかったが、PI3K/mTOR同時阻害剤BEZ235添加時には放射線感受性が増強し、細胞死が有意に多く誘導された。さらに、mTORの下流分子であるHIF-1αが放射線照射後に発現上昇し、siRNAにてノックダウンした場合にも、放射線照射による細胞死の割合が有意に上昇した。
【結論】TP53変異は子宮体癌細胞における放射線抵抗性を促す因子であることが示された。放射線照射により活性化される経路の一つであるPI3K/mTOR経路の阻害、さらにはその下流のHIF-1αの阻害が放射線感受性を高める可能性があり、今後の臨床への応用が期待される。

Reason

予定通りに研究を遂行できており、研究の成果を英語論文として投稿し、Gynecologic Oncology誌に採択された。その他にも、子宮体癌において細胞死・増殖抑制・予後に関わる分子を同定し、British Journal of Cancer, FEBS Letters, Gynecologic Oncology誌に採択された。卵巣がんにおいても治療感受性・予後と関連するサブグループを同定し、PLoS One誌に報告した。H27年度のAmerican Association for Cancer Research (AACR) Annual Meetingをはじめ、国内外の学会発表の場でも研究成果を報告した。以上の通り、本研究プロジェクトは順調に進展している状況である。

Strategy for Future Research Activity

平成27年度は、子宮体癌において放射線感受性を増強するための標的分子として、PI3K/mTOR/HIF-1α経路を同定した。H28年度以降においても、これまでの成果をもとに子宮体癌・卵巣癌におけるアポトーシスを誘導する新規分子標的治療法の探索研究を遂行していく予定である。
今後については、以下の２つのプロジェクトを中心に研究をさらに推進していく予定である。
一つは、子宮体癌において細胞生存（抗アポトーシス）に関わる分子を明らかとし、既存薬を活用することで、アポトーシス誘導効果を示す新規治療法を見出していくことである。予後や治療感受性との関連についても引き続き検討し、子宮体癌の分子病態メカニズムの解明をはかる。
もう一つは卵巣癌におけるアポトーシス誘導能を有する分子標的治療法の探索である。卵巣粘液性腺癌や明細胞腺癌といった、抗癌剤感受性の低い組織型を中心に、平成27年度に引き続き研究を進めていく予定である。
