| Research Abstract |
免疫担当細胞である単核球は種々のアラキドン酸代謝産物やサイトカインを分泌し自己免疫性慢性炎症性疾患の発症進展に密接に関与しているとされている。魚脂多価不飽和脂肪酸の一種であるエイコサペンタエン酸(EPA)には抗炎症括免疫作用があるといわれており,我々は自己免疫性慢性炎症性疾患である慢性炎症性腸疾患(潰瘍性大腸炎,クロ-ン病)や乾癬におけるサイトカイ産生能と疾患の活動性との相関及びEPAの治療効果について検討した。
1)モルモットにカラニゲン誘発性潰瘍性大腸炎を発症させると,肓腸部位を中心として粘膜潰瘍と多数の単核球の集績がみられ,モルモットも粘血便を排出する。これに対してEPAを経口投与させると,粘血膜の排出が著明に減少し,腸粘膜の潰瘍,単核球の疫潤及び腸粘膜よりのLiB_4,TxA_2及び ^DGE_2の産生も有意に抑制された。
2)潰瘍性大腸炎及びクロ-ン病の患者の末梢血の単核球でのインタ-ロイキン1β(エレ-1β),インタ-ロイキン2(エレ-2)及びインタ-ロイキン6(エレ-6)の産生は増加しており,これらサイトカイン産生脂と疾患の活動性には正の相関がみられ,疾患が治療により改善すると共にサイトカイン産生能も低下した。
3)乾癬に対するEPAの長期投与の治療効果について検討した。乾癬のの患者にEPA3.6g/日を投与すると投与后3ー6ケ月で皮フ症状の明瞭な改善と,好中球材能の抑制及びLTB_4の産生低下がみられた。
以上の成綾はいづれもEPAに抗炎症抗免疫作用があることを支持する成綾であり,EPAが栄養学的及び薬理学的観点から薬用であることを裏付けるものである。
