Research Abstract |
1. Incidence of pancreaticobiliary maljunction (PBMJ) in Niigata district. Incidence of high amylase activity (>10.OOOIU/L) in gallbladder bile among cholecystectomy cases was 2.3% (7/305) and incidence of PBMJ among ERCP cases was 2.7% (63/2336). These results suggests that incidence of PBMJ in Niigata is not paticularly high comparing to other parts of Japan. Therefore, it is not likely that PBMJ plays an important role in the high incidence of galllbladder cancer in Niigata. 2. Bile acids and fatty acids in gallbladder bile. Bile acid concentration in gallbladder bile decreased prominently in patients with gallbladder cancer and those with cholesterol stone in a lesser degree. Ratio of secondary to primary bile acids increased in gallbladder cancer. Disturbance of concentrating function of the gallbladder seemed to precede to cystic duct obstruction or mucosal devastation secondary to carcinoma. Fatty acid concentration was also very low in gallbladder cancer. However, individual difference in fatty acid concentration is so large that meaning of this change must be evaluated after accumulation of much more number of cases. 3. Mutagenecity of gallbladder bile. Preliminary results of Ames test of gallbladder bile showed slightly lower incidence of mutagenic bile in recent cases. To confirm this tendency, it is necessary to follow longer in much more cases.
