Research Abstract |
An increase in anti-Klebsiella antibodies in active ankylosing spondylitis (AS) patients and anti-Proteus antibodies in rheumatoid arthritis (RA) patients have been reported in Europe and America. We have investigated lgG,lgA and lgM class specific antibodies to three bacteria (K.pneumoniae, P.mirabilis and E.coli) and three synthetic peptides (HLA-B27 peptide, pullulanase-D peptide which contains a sequence crossreacting with HLA-B27 and a control scrambled peptide) by enzyme-linked immunosorbent assay in Japanese patients with AS and RA.Serum samples from 152 Japanese subjects were tested. The 29 active AS patients had elevated lgA antibodies to K.pneumoniae whilst the 30 active and 20 probably active RA patients showed elevated lgG antibodies to P.mirabilis. No elevation in any antibody class against E.coli was shown in any group. AS patients had specific auto-antibodies against the HLA-B27 peptide. Active AS patients had lgG and lgA antibodies against the pullulanase-D peptides, whilst no such elevation was present in inactive AS or RA patients. No such elevation in any antibody against the control peptide was observed in the disease groups when compared to controls. These results proveide the first evidence of AS and RA patients in Japan having specific elevations of antibody to K.pneumoniae and P.mirabilis respectively. This suggests that the presence of K.Pneumoniae in AS patients and P.mirabilis in RA patients may play an important role in triggering and/or exacerbating these diseases in many different parts of the world.
