Outline of Final Research Achievements

CT-Z-score mapping is a promising tool for detecting hypoattenuation area of acute cerebral infarction with increased z-score. Z-score is increased in hypoattenuation lesions representing early ischemic changes in middle cerebral artery stroke. Early gray and white matter edema could be well visualized at CT-Z-score mapping. Corresponding to high z-score area, cerebral blood volume was decreased on CT perfusion.

Academic Significance and Societal Importance of the Research Achievements

急性期脳梗塞において血栓溶解療法や血栓回収術の適応を考慮して画像診断を行う際、短時間に検査を施行できる単純CTを用いて早期虚血変化を的確に評価することは重要であるが、本研究でCT・Zスコアマップが急性期脳梗塞の検出の補助診断ツールとしての有用性を示したことは今後、解析ソフトの流布や技術的改良が相まって汎用化し、波及効果が期待できる点で社会的意義がある。また、CT低吸収変化のみられた領域は非可逆的虚血領域であると従来、考えられてきたが、可逆的早期浮腫の変化も単純CTの画像情報に含まれていることは新たな知見であり、学術的に意義深い。
