| Outline of Final Research Achievements |
We have undertaken these studies, since clinical studies on water-soluble vitamins have been limited. In the elderly, muscle strength was associated with hyperhomocysteinemia. In patients with Crohn's disease, serum vitamin B12 level was decreased despite intake higher than recommended daily allowance, suggesting impaired absorption. In patients scheduled for or after gastrectomy, serum vitamin B12 level was decreased, suggesting that vitamin B12 deficiency occurs much earlier than previously considered.

| Academic Significance and Societal Importance of the Research Achievements |
ビタミン欠乏より軽度の不足であっても、種々の疾患リスクが増加するが、水溶性ビタミンに関する臨床研究は、わが国においてきわめて乏しい。本研究において、ビタミンB12不足は高齢者の筋力低下と関連することを明らかにし、クローン病モデルにおいて、吸収障害がビタミンB12不足の重要な原因となることを示した。さらに胃切除患者において、従来考えられているより、はるかに早期から、ビタミンB12不足が起こることを明らかにした。すなわちビタミンB12不足は、慢性疾患のリスクと関係し、不足者の割合は高い。このようなビタミンB12不足の臨床的意義を明らかにすることは、高齢化社会において、大きな社会的意義を持ち得る。
