Outline of Final Research Achievements

We have developed a rubric to assess the performance of first- and second-year Japanese medical students taking patient histories in English. Our rubric consists of the following dimensions: 1. Spoken English Proficiency (SEP) to evaluate fluency and accuracy in speaking English; 2. Communication and Interpersonal Skills (CIS) to evaluate empathy for building rapport with patients; and 3. Sequence of Questions (SOQ) to evaluate responsiveness for digging into a patient's story. The first two sections were based on scoring subcomponents used in the USMLE Step 2 Clinical Skills, and the third section was especially designed for the purpose of assessing pre-clinical medical students. SOQ only focuses on data-gathering skills, without requiring mature clinical knowledge that is necessary to narrow down a possible diagnosis.

Academic Significance and Societal Importance of the Research Achievements

国際化が進む近年,英語での医療面接ができることは日本の医学部を卒業する全学生に求められている。日本語での医療面接を含む客観的臨床能力試験への指導は,医学知識が身につく4年生以降に行う医学部が多い。
その一方で,英語医療面接に必要な,英語コミュニケーション,医師としてのマナー・視線・心構えなどの習得には時間がかかり,1年生から繰り返し指導を行うのが効果的で,英語教師が活躍できる場面も多い。
こうした英語医療面接の実技指導に役立つツールとして評価ルーブリックがあり,学習者に到達目標を明示する教育上の役割も果たす。第2言語用でコミュニケーションに焦点を置いたものが確立されていなかったので開発に着手した。
