Outline of Final Research Achievements |
Our study showed that "acute stroke" is more strongly related with high residual on-treatment platelet reactivity (HRPR) on antiplatelet therapy than genetic polymorphism which is most known. In acute stroke patients, the release of phosphorylated-heat shock protein 27 (HSP27) from thrombin-stimulated platelets is accelerated via Rac.

Academic Significance and Societal Importance of the Research Achievements |
従来、不応症の主な原因は薬剤相互作用や薬物代謝酵素の遺伝子多型とされ、抗血小板薬の変更や追加投与により対処せざるを得ないと考えられていたが、急性期脳梗塞における一時的な不応症においては、血小板におけるトロンビン刺激感受性の亢進、および血小板から放出されるHeat Shock Protein 27の関与が示された。 急性期においては、トロンビン受容体拮抗薬などの併用が臨床的効果をもたらす可能性が示唆された。
