Outline of Final Research Achievements |
Total leukocyte (including granulocytes, NK cells, T cells, B cellsand monocytes) migration increased as labor progressed (spontaneous TLvs TNLp<0.008). More leukocytes migrated at PTL than at PTNL withintact membranes (p<0.0001). Receiver operating characteristic curveparameters indicated the cut-off point for delivery within 7 days to be37,082 leukocytes with sensitivity 78.1%, specificity 88.9%, positive pre-dictive value 91.4%, negative predictive value 72.7%, and area under thecurve 0.83.The LMA has the potential to become a much needed point-of-care clinical test to predict delivery within 7 days.

Academic Significance and Societal Importance of the Research Achievements |
分娩に至る機序が妊娠32週以前と正期産とで異なる可能性があるが、少なくとも妊娠後期から分娩においてはchemoattractantに対する白血球の反応性は徐々に高くなっていることが予想された。これらの反応性を評価することで分娩誘発が成功するかどうかや急産の予測、また予定帝王切開前の陣発の予測などの臨床応用ができる可能性が示唆された。今後は、chemoattractantのなかで何に対して白血球が遊走しているのかについて研究を継続し、臨床応用の可能性について引き続き探求していく予定である。
