Outline of Final Research Achievements

We demonstrated that open lung approach during mechanical ventilation protects lungs of lipopolysaccharide-induced experimental ARDS model independtly from reduction of driving pressure. Moreover, increasing oxygen concentration during mechanical ventilation could not alleviate the atelectasis-induced lung tissue hypoxia and lung tissue injury.

Academic Significance and Societal Importance of the Research Achievements

本研究によって,ARDS患者の人工呼吸中に無気肺を減らすことそのものがDriving Pressureの減少とは独立して肺保護効果を持つことが初めて示された.一方で,吸入酸素濃度を増加させても無気肺に伴う肺胞低酸素及び組織傷害を軽減できないことが示唆された.以上から,ARDSの人工呼吸管理においては,無気肺を減少させるとともに,吸入酸素濃度はできる限り減らすことが有益である可能性が示唆され,今後の臨床研究の方向性に示唆を与えることができたと考えられる.
