Outline of Final Research Achievements |
Cardiac presynapse sympathetic nervous function was investigated using cardiac positron emission tomography (PET), with carbon-11 hydroxyephedrine (C-11 HED) in patients with chronotropic incompetence (CI) without heart failure. Presynapse sympathetic nervous function was quantitatively evaluated using HED myocardial retention rate (HED-RI) on HED PET. The HED-RI was significantly higher in patients with in patients with CI (n = 9) than in healthy subjects (n=6) (13.8 ± 3.0 vs. 8.5 ± 3.9%/min, p = 0.01). This indicates that in patients with CI, cardiac sympathetic nerve terminals cannot adequately release catecholamines, which may partly be involved in the mechanism of CI.

Academic Significance and Societal Importance of the Research Achievements |
変時性不全におけるpost-synapse機能の障害はすでに確立している。これのみであればβ受容体への介入手段としてβ遮断薬が考慮される。本研究においては、これまで検討されてこなかったpre-synapse機能を評価の対象とした。その結果、変時性不全においては従来指摘されてきたpose-synapse機能の障害のみならず、pre-synapse機能においても障害が存在することが証明された。このため、変時性不全においては、pre-synapse機能の障害を補う必要があることが明らかになった。本研究の結果は、変時性不全に対する適切な介入方法の創造に寄与できる。
