Outline of Final Research Achievements

We built a tele-nursing system (T-SCOT) for cancer patients undergoing outpatient chemotherapy, and implemented and evaluated T-SCOT with the aim of preventing the exacerbation of symptoms and improving quality of life (QOL). Consent was obtained from 12 subjects, including 8 men (66.6%) and 4 women (33.4%), with a mean age of 68.6 ± 5.2 years. A comparison of MDASI-J and QOL-ACD three months after intervention in the intervention group and control group showed no significant difference in MDASI-J. However, in QOL-ACD, the intervention group had significantly higher scores in mental and psychological state (p=0.02) and total QOL score (p=0.01), and QOL was high. QOL was thought to be high because the intervention group could visualize and confirm their own symptoms anytime and anywhere, felt reassured from being watched by medical professionals every day, and receiving timely responses from medical professionals.

Academic Significance and Societal Importance of the Research Achievements

開発した遠隔看護システムは、血圧測定や体温・SPO2など測定した値がBluetoothを利用して自動的にクラウドへデータが保存される機能を付与させるなど、高齢者の多いがん患者のICT操作の負担感を軽減できる。これによりがん患者は長期間継続的にICTを活用し、自身の行動変容につなげることができた。
看護師は遠隔看護を通して患者との関わりが密になれば患者の主体性や行動変容に対する関心を高める効果があり、主体的療養行動の支援者としての看護師の役割を再認識することができた。今後外来で治療を行いながら自宅で療養している患者に対してタイミングのよい支援を構築できたことは本研究の持つ意義は大きいと考える。
