Outline of Research at the Start

経カテーテル大動脈弁留置術(TAVI)は、高齢者など手術リスクの高い重症大動脈弁狭窄症患者を対象とした低侵襲治療であり、その後の心不全発症を防ぐことで、医療費の総額が抑制されることが期待されている。我々はこれまで、心不全患者で、高精度センサーカテーテルを用いて左室拡張能(時定数tau)を計測し、左室拡張機能障害が進行した症例では、治療介入の効果が限定的であることを報告してきた。本研究では、TAVIが成功しても心不全を発症するリスクの高い症例について、左室拡張機能の観点からその病態を明らかにし、TAVIの患者選択基準や治療介入のプランニングについて再検討していく予定である。

Outline of Final Research Achievements

Left ventricular (LV) diastolic stiffness, a parameter indicative of LV diastolic function, was calculated using cardiac catheterization data from patients undergoing transcatheter aortic valve implantation (TAVI). Patients with higher LV diastolic stiffness exhibited more adverse cardiac events, and multivariate analysis demonstrated its status as an independent prognostic determinant. Furthermore, echocardiographic data yielded the left atrial volume (LAVi)/late diastolic mitral annular tissue velocity (a') ratio, reflecting left atrial remodeling, and the tricuspid annular plane systolic excursion (TAPSE)/pulmonary artery systolic pressure (PASP) ratio, indicative of right ventricular function, both identified as prognostic factors.

Academic Significance and Societal Importance of the Research Achievements

高齢者における重度大動脈弁狭窄症では、左室への長期間にわたる圧負荷により、左室拡張能だけでなく、その上流に位置する左房や右室も既に障害を受けている可能性がある。経カテーテル大動脈弁留置術(TAVI)は、高齢者を含む高リスク患者を対象とした低侵襲治療である。しかしながら当研究の結果、左室のスティフネス増加、左房リモデリングの進行、右室機能低下などの要因が併存する場合に、TAVI治療に成功してもその後心臓死や心不全入院を起こしやすいことが示された。これらハイリスク症例を同定し、早期の治療介入や適切な薬物治療を行うことによって、TAVIは健康寿命の延伸だけでなく医療費削減にも貢献すると考えられる。
