Outline of Research at the Start |
食道癌に対する放射線治療において、治療後数カ月から数年後に発症する心臓障害が問題となっている。本研究の目的は、根治が期待できる食道癌患者に対して高精度放射線治療で心筋への線量を低下させる事で、有害事象軽減につながるか前向きに検討することである。①心臓MRIによる画像での心筋変化、②心電図・心臓超音波・採血データの経時的変化、③心嚢液貯留の経時的変化、④心事故の有無などを評価していく。

Outline of Final Research Achievements |
In a prospective clinical trial (UMIN000038147) to examine myocardial damages using cardiac MRI in intensity-modulated radiotherapy for esophageal cancer, cases were collected from patients with early esophageal cancer. Due to the impact of the COVID-19, only 7 patients were registered in the present study. All patients completed their scheduled chemoradiotherapy. The dose constraint of left ventricle was met in all patients. Of these, 6 patients underwent MRI after 6 months of treatment. We confirmed that there were no abnormal findings corresponding to the radiation field. At this time, no cardiac events have not been observed. We intend to conduct detailed analysis in the future. We will compare these results in the present study with those in patients who underwent three-dimensional conformal radiotherapy in our past study. We intend to examine whether there would be any difference in the incidence of myocardial damages and cardiac events.

Academic Significance and Societal Importance of the Research Achievements |
近年、食道癌に対する放射線治療後の晩期有害事象として心臓障害が問題となっている。我々は過去の研究において、食道癌放射線治療後半年後にMRIで放射線心筋障害を認めた症例において、その後の心事故の発症頻度が高くなる事を示している。本研究において、早期食道癌症例に対して強度変調放射線治療を施行する事で、心筋への線量を低減する事が可能であり、放射線治療後半年後に施行したMRIでも照射部位に一致した異常所見も認めなかった。今後は更なる解析が必要となるが、本研究の結果は長期生存が期待できる食道癌に対して、心事故の発症リスクを減らす手段として有用である可能性がある事が示唆された。
