Outline of Research at the Start

根管逸脱、側枝、イスムスを有した根管に対するLAI根管洗浄の評価のため、まずLAIの光子誘導励起光音響流を粒子画像流速解析する。また、LAIについては根尖孔外に生じる圧力や、光子誘導励起光音響流の流速強度分布、速度また複雑な根管における洗浄効率や、側枝への洗浄圧や側枝外への圧力を測定し、その機序を解明する。さらにそれぞれ根管逸脱部の位置、イスムス、フィンの位置を変化させた条件で、根尖孔外に生じる圧力、蒸気泡挙動、洗浄効率、殺菌効果を総合的に測定する。

Outline of Final Research Achievements

In recent years, a new root canal cleaning method (Laser Activated Irrigation hereafter LAI) has been devised, and although it is expected to contribute to improving treatment results by a high cleaning effect on details, a study reporting the details has been reported. rare. Not only the cleaning effect but also the risk of overflow to the outside of the apical foramen should be considered, and the purpose is to improve the root canal cleaning effect and improve the treatment results for infected sites that were difficult to clean in the past. , The cleaning effect of LAI in various root canal forms will be analyzed hydrodynamically under the condition of reproducing the complicated structure of the root canal, and the details such as safety, bactericidal property, and new cleaning method will be comprehensively examined.

Academic Significance and Societal Importance of the Research Achievements

LAIは、根管内に高速の水流、衝撃波を発生させ、キャビテーションが生じることから高い洗浄効果が期待できる。LAIは、SI、PUIに比べ、根管内のデブリの除去効率が同程度、あるいは有意に高いことや、象牙細管内の細菌を減少させることが報告されている。しかしながら、LAIの利点を最大限に活用が期待される複雑な根管形態と洗浄する位置による影響に着目し、LAIの流体力学的流量分布および殺菌能を比較検討した研究は未だない。本研究はより臨床に重点を置き、LAIの効果を検討するものであり、根尖側の清掃が実施不可能な症例、つまり従来法のみでは保存困難になる可能性のある症例の臨床的解決を特に見据えた研究である。
