Outline of Research at the Start

日本人の主要な死因に悪性新生物と共に心疾患、脳血管疾患が含まれており、これら動脈硬化性疾患の予防の必要性はますます高まっている。近年、歯周病と動脈硬化との関連が議論されているが、咀嚼能力との関連についてはほとんど検討されていない。また、多くの口腔健康と動脈硬化との関連についての研究は横断研究であり、その因果関係については明らかとなっていない。そこで本研究は、都市部一般住民のランダムサンプルを対象とした前向きコホート研究より、歯周病と咀嚼能力の低下が動脈硬化に及ぼす影響、さらには脳卒中、心筋梗塞の発症との関係を縦断研究で明らかにすることを目的とする。

Outline of Final Research Achievements

This study assessed the relationship between oral health and general health in Japanese urban population. Below results were found.
1. Decreased masticatory performance may be a risk factor for developing metabolic syndrome in men. In addition, for each component, a significant association was found between masticatory performance and high blood pressure, hypertriglyceridemia, and high blood glucose in men. 2. Progression of periodontal disease may be a risk factor for decreased masticatory performance. 3. Smoking may be a risk factor for decreased masticatory performance.

Academic Significance and Societal Importance of the Research Achievements

近年、歯周病と動脈硬化との関連が議論されているが、咀嚼能力との関連についてはほとんど検討されていない。また、多くの口腔健康と動脈硬化との関連についての研究は横断研究であり、その因果関係については明らかとなっていない。今回の縦断研究より、咀嚼能率の低下がその後のメタボリックシンドローム発症の危険因子となる可能性が明らかとなった。また、一方で、歯周状態の悪化、喫煙が、咀嚼能率低下の危険因子となる可能性が示唆された。以上より、歯科での歯周病予防に加え禁煙指導が咀嚼能率低下を予防し、咀嚼能率低下を予防することにより、メタボリックシンドロームの予防に寄与する可能性が示唆された。
