Outline of Research at the Start

本研究の目的は、「欠測データ解析の枠組み」から医療経済評価でのマッピングを捉え直すことで、医療経済評価でのバイアスの観点からマッピング手法が満たすべき条件を記述し、今後のマッピング手法の開発や選択に対する実践的な指針を得ることである。そのために、マッピング手法が医療経済評価でのバイアスの観点から妥当であるための条件を導出し、それをシミュレーション実験で例示し、実データを用いた実証研究で既存のマッピング手法がその条件を満たすか評価する。

Outline of Final Research Achievements

I derived conditions for unbiased estimation of expected health state utility values through mapping when conducting health economic evaluations using the framework of missing data analysis. I developed direct and indirect mapping algorithms from EORTC QLQ-C30 onto EQ-5D-5L index. Additionally, I showed little impact of the order of measuring source and target scales on the mapping algorithm using real-world cancer data. Furthermore, I explored gradient boosting tree for mapping EORTC QLQ-C30 onto EQ-5D-5L index but better results were not obtained than regression models. I proposed a sample size calculation method for linear regression-based mapping algorithms.

Academic Significance and Societal Importance of the Research Achievements

マッピング手法の新規開発やマッピング手法の医療経済評価での利用に対して本研究で得られた条件は、今後のマッピングに関連する研究が向かうべき適切な方向性を示し、医療経済評価でのマッピングの実践を改善することにつながると期待される。
開発したマッピングアルゴリズムは、今後の医療経済評価で利用かのうである。
開発した必要サンプルサイズ計算法は、線形回帰モデルを用いる場合にしか現在は用いることができない限界がある。しかし、この研究をきっかけに、マッピング研究に用いられるより複雑な回帰モデルでのサンプルサイズ計算方法の開発が行われることが期待される。
