| Outline of Research at the Start |
少子高齢化が進む我が国では、在宅医療を含むプライマリケアの確立に向け、患者が医療機関を退院した後も、在宅・介護施設など地域で安心した生活を送れるよう、医療機関と介護施設や在宅療養を担う施設と切れ目のない関係を築くことが求められている。
本研究では、患者が心身ともに安定した在宅療養を続ける上で欠かせない「適切な食事管理」に焦点を当て、長期療養が必要な慢性疾患患者に対し、退院前にどのような支援（内容・手法）を行うことが、適切な食事管理の遂行につながるか、検討を進める。
具体的には、患者・家族（ケア担当者）の生活状況や、ニーズに焦点をあてた質的検討を行い、実情に即した支援の在り方を提案することを目指す。

| Outline of Final Research Achievements |
This study aimed to explore the collaboration between healthcare professionals, such as registered dietitians, and patients during hospitalization to determine dietary habits after discharge while considering the patient's values and lifestyle. Two surveys were conducted including a web survey targeting registered dietitians involved in discharge support and a self-administered questionnaire targeting community-dwelling elderly people including outpatients. The results allowed us to explore optimal support during discharge planning that included a sufficient SDM (shared decision making) process regarding dietary habits. Results were presented at an academic conference and submitted for publication and allowed for the development of a tool (booklet) that incorporates the SDM process at the time of hospital discharge.

| Academic Significance and Societal Importance of the Research Achievements |
本研究では、近年、高齢化が進み在宅療養の拡充が求められる中、慢性疾患で継続的な食事療法が必要な入院患者に対し、患者・家族の価値観やライフスタイル等の実情を踏まえ、入院中からどのような支援を行えば、退院後も適切な食事管理を継続できるかを検討した。実際、退院時食支援を中心的に担う管理栄養士側、その対象となる側の双方の食支援に関する実態・ニーズを把握したことで、現状の課題を整理が進み、実際に入院医療機関で退院時食支援に活用できる支援ツールの開発、提案へとつなげた。
